A Service Agreement is an agreement between you and our services that clearly outlines the services you will receive. This is different to your Support Plan. Your Service Agreement includes:
- the terms and conditions of your therapy
- the rights and responsibilities that pertain to both you and us
- our complaints procedures
- our cancellation policy
- the budget for your therapy
- where and when you will receive therapy
- what to do if you have a problem or wish to make a complaint
